Registration, Forms, and Documents
Children are eligible to enroll in Kindergarten if their fifth birthday falls on or before December 1st.
A birth certificate, proof of immunization and residency are required.
Registration and screening for children entering Kindergarten in September will be held in the spring.
Mrs. Kazek is available to help you register between 7 a.m. and 2:00 p.m. during the summer months and 7:30 a.m. to 3:30 p.m. each weekday during the school year. For information or to make an appointment, please call 942-9200 ext: 1800.
PARENT/GUARDIAN:
Plan ahead! The following documents are REQUIRED for each student being enrolled:
- Birth Certificate
- Immunization Records
- If in foster care: a DSS-2999 form
- Custody decrees bearing a judge's signature, if parents are divorced OR if student resides with someone other than the biological parent (i.e. aunt/uncle/grandparent, other family member, etc.)
- Proofs of Residency (upon request District officials will ask you to furnish verification that the address listed on enrollment forms is your actual place of residence). It is always a good idea to bring along proof at the time of enrollment.
Please do NOT mail or fax any documents. You must provide all information in person.
Registration can be completed IN PERSON during the hours from 7:30 am to 3:30 pm
This is an important time in your child's life - please allow at least 30-45 minutes per child for completing the necessary paperwork.
